Information about PCOC

The Palliative Care Outcomes Collaboration (PCOC) is a national program that works with palliative care services across Australia to understand people’s experiences and improve the quality of care provided. PCOC uses standardised assessment tools to help care teams better understand symptoms, concerns, and priorities for patients, their families, and carers.

As part of the program, your care team may ask you to complete short assessments about how you’re feeling. These tools help track changes over time and support care planning that reflects what matters most to you.

 

What This Means for You

  • Your experiences guide your care. The information you share helps your care team respond to your needs and tailor support to your goals and preferences.
  • Clearer communication. Regular assessments support meaningful conversations between you, your family, and your care providers.
  • Continuous improvement. The information collected helps participating services understand what is working well and where improvements can be made for current and future patients.

 

For Families and Carers

Families and carers are an important part of the care journey. PCOC includes tools that help the care team understand your experiences and needs as well. Your insights contribute to planning care and ensuring support is available for you too.

 

Your Participation

Taking part in PCOC assessments is voluntary. The information you provide is confidential and used to support your care and to help improve and strengthen palliative care services across Australia. If you have questions or prefer not to participate, your care team will respect your choice.
