HDR Forms

Commencement Discussion Checklist: (previously called Commencement of Candidature): Facilitating discussion between HDR candidates and supervisors regarding mutual expectations, roles and responsibilities, candidature requirements, available support, and other key matters that should be discussed during the early stages of candidature. Completion of this checklist is a compulsory milestone and must be undertaken in consultation with your supervisor(s) within the first six months of candidature.
HDR Space Allocation Request Form: Complete after discussing with your supervisor to request shared office space, laboratory access and access cards
Application for Credit for Prior Learning: Seeking recognition for a similar coursework subject completed before

Research Proposal Review (RPR) Evaluation Form: Completed by the panel who evaluate your research proposal review

HDR Allowance Expenditure Request Form: Organising some funding to support your research

Application for Leave of Absence: Suspending studies for one session
Course Transfer Request Form: Transferring to a different HDR course, changing the delivery mode of the current HDR course, or changing campuses, while remaining under the same Principal Supervisor
Application for Late Withdrawal without Academic Penalty: Withdrawing from a coursework subject for reasons beyond your control
Change of Supervisor Form: Changing supervision panel
Reduced Study Load Form: Changing study load

Domestic Postgraduate Tuition Award Application: Overtime domestic HDR candidates applying to extend candidature for one session without paying fees

Nomination of Examiner Form: Before submitting thesis, your principal supervisor must complete this form in consultation with you
Head of Postgraduate Studies - Thesis Reporting Form (PhD & MPhil): When submitting thesis, the Head of Postgraduate Studies will complete this form
Head of Postgraduate Studies - Thesis Reporting Form (MRes): (For Master of Research candidates)
Online HDR Thesis Submission: Submitting your actual thesis
Agreement for the Deposit of HDR Thesis in Digital Repository: Complete when submitting your final thesis
Research Career Launch Scholarship: Completed by UOW Australian Government Research Training Program (AGRTP)/University Postgraduate Award (UPA) holders who have not exhausted all scholarship stipend payments at the time of submission of thesis
