Submitting applications and forms

These forms and applications are for current students. If you can't find the form you're looking for, try searching AskUOW using your student details.

Each application or student form will have specific information on the process and requirements for submitting.  If you are arranging for a third party to collect documentation on your behalf, an Authorisation Form must be completed in SOLS.

Official documentation

Form Deadline
Academic transcripts  No cut off date. Will show only subjects with grades released to date.
AHEGS Available after degree is conferred (for degrees conferred December 2010 onward).
Replacement testamur Available after degree is conferred, if original is lost or damaged or if name has changed.

To add a subject, last day to add subject with approval, as per session key dates

Provisional waiver application (apply in SOLS) Check SOLS homepage for open and close application dates (normally prior to the session census date, as per key dates (more information)

Course related forms

Form Deadline
Reduced study load application To avoid financial liability, prior to the census date for the session, as per key dates.  
Course transfer form Course transfer applications should be submitted at least 3 weeks before the start of session.
Declare or change major or minor form Apply as soon as your major/minor is decided. Can apply anytime but must finalise prior to conferral of degree. Specific dates will be given when applying to graduate.
International studies minor form Apply as soon as you decide to take the minor. Can apply anytime but must finalise prior to conferral of degree. Specific dates will be given when applying to graduate.

Credit for prior learning - Formal Learning Coursework

Credit for prior learning - Informal and Non-formal Learning Coursework

If you are currently enrolled in any subject/s for which you are seeking credit, you must submit your application no later than 3 weeks before the start of session.

To allow time for you to make decisions about what subjects you will need to enrol into for upcoming sessions, you should submit your application as soon as possible (more information).

Credit for prior learning - HDR Can apply anytime but must finalise prior to conferral of degree. Specific dates will be given when applying to graduate (Contact Graduate Research School).

Taking a break from study

Leave of absence - Undergraduate domestic (apply in SOLS - student forms) Census date as per key dates 
Leave of absence - Undergraduate domestic 12 months leave already taken Census date as per key dates
Leave of absence - Postgraduate Domestic Census date as per key dates
Leave of absence - Higher Degree Research (domestic and international) Census date as per key dates
Leave of absence -  International Census date as per key dates

International student forms

COE extension (apply in SOLS) No earlier than three (3) months prior to your visa expiration date (more information)
Letter of release  To avoid financial liability, prior to the census date for the session, as per key dates 
Course withdrawal application  To avoid financial liability, prior to the census date for the session, as per key dates

Fees and money

Form Deadline
Appeal against late fee No cut off date.  Must be settled to avoid results and documentation withholds (more information)
Refund of fees application (Domestic students) No later than 12 months after withdrawal of subject or last day of session, as per key dates - (more information)
Refund of fees application (International students) No later than 12 months after withdrawal of subject or last day of session, as per key dates - (more information)
Request to transfer credit form (to UOWC or accommodation) No cut off date.

Cross institutional and non-award study

Form Deadline
Cross institutional study at UOW form Three (3) weeks prior to start of session, as per key dates.
Cross institutional study at another university Check with University in which the application is intended for submission.
Non-award study at UOW form Three (3) weeks prior to start of session, as per key dates.
