| Academic Approval | Use this form to add a subject after SOLS enrolment has been closed for the session and/or exceed session credit point limits and/or waive provisional status on a subject(s). | |
| Application for Credit or Exemption | Use this form if you are seeking credit or exemption (Advanced Standing) for subjects studies at another education provider. | |
| Application for Leave of Absence | Use this form if you wish to take leave from your study. Leave is not permissible in your first session of study. You can not take more then one years worth of leave over the duration of your study. View the General Course Rules for details. | |
| Application to Vary Course Registration (Course Transfer) | Use this to change your course of study. View the policy here. | |
| Change to Personal Details | Use this form to change your personal details recorded by the University i.e. change of name. You may change your address details in SOLS. | |
| Course Withdrawal | Use this form if you do not wish to continue your study. | |
| Declare or Change Major | Use this form if you wish to change the current major you are studying. e.g B.Com Major from Marketing to Accounting. | |
| Late Withdrawal without Academic Penalty | Use this form to withdraw from a subject. Under exceptional circumstances you can apply to withdraw late without academic penalty. View the policy here. | |
| Application to Graduate - Offshore | Application to have your degree conferred by the Vice Chancellor (degrees conferred by the Vice Chancellor will be sent to the Offshore Partner for distribution to graduates). Use this form if you do not wish to attend a Graduation Ceremony or if you wish to attend the Graduation Ceremony held at the Partner Institute through which you have been studying your University of Wollongong degree. | |
| Application to Graduate - Wollongong Ceremony | Use this form only if you wish to attend the Graduation Ceremony held at the University of Wollongong Campus in Wollongong, Australia. |
Returning Your Forms
Completed forms should be returned to the Administration Unit at the Partner Institute you are studying your degree with.
