 1. Form Information

 1.1 Accessing Forms

The tables on this page state the name of the form, it's purpose and how it can be accessed. The three ways a form can be accessed are described below;

  • SOLS - Any forms that are made available on SOLS can only be accessed by logging into your personal SOLS and are completed and submitted online.

     
  • PDF - Any forms in this format can be accessed via this site but must be submitted via mail or personal delivery to Student Central. See Returning Your Forms. To access and print PDF documents you must have Adobe Reader.

     
  • Link - This means that the information you are looking for is provided via an external web page link.
 1.2 Due Dates

The due dates for forms can vary depending on the session in which you are enrolled. To see dates regarding academic approval please log into SOLS and select 'Enrolment Variation'. Also refer to the specific instructions provided for each form as well as familiarising yourself with the University's Key Dates.

 1.3 Address Management

Where letters are able to be sent directly to your address you must ensure that the address provided in SOLS is correct. Find out how to manage your address through SOLS here.

 1.4 Personal Details Management

Personal Details can be changed in person at Student Central with original or certified documentation.
Preferred first names can be added via an online form in SOLS.

 2. Enrolment Forms

 2.1 All Students

Academic Approval - Subject AdditionUse this if you need to add a subject late -between week 2 of session and the 'Last day to add subject with approval of Academic Adviser & Head of Department' (view session key dates here); or want to exceed your credit point limit.  Please note this does not apply to Intakes.
Academic Approval - For Waiver of Provisional StatusThis form is for students who wish to have the pre/co-requisites waived for a subject in which their enrolment status is provisional.PDF
Application to Amend Academic RecordUse this form if there has been an enrolment error where you need to remove a subject if you did not attempt the subject; or to add a subject not formally enrolled to have a grade declared. Applications can be received no later than four (4) weeks after the release of results. Please note fees may apply.

Intervention Strategy Form This form is to be used by both the student and Course Progress Advisors for the determination of an intervention strategy.      PDF
Application for Declare or Change of Intended Major or MinorFor those students who wish to change the current major they are studying. e.g. To change your B.Com major from Marketing to Accounting.PDF
Application for Late Withdrawal Without Academic PenaltyYou can apply to withdraw late without academic penalty after the end of the ninth week of session (for subjects in a standard session) or after the last day to withdraw without academic penalty (for subjects in a non-standard session) and before the end of the last teaching week of the session under exceptional circumstances. Application for Leave of Absence - Domestic Students

For students who wish to take leave at the beginning of the second session of registration. View the policy here.

Academic Transcripts & AHEGS Order FormFor students who wish to have an official academic transcript or Australian Higher Education Graduation Statement (AHEGS) printed detailing their course marks, grades and awards.

Application for Replacement TestamurFor those students who wish to request a replacement graduation testamur where the original has been lost/damaged/stolen, due to change of name, or due to damage. 
Authorisation FormThis form is used to nominate another person to collect items from Student Central on your behalf.PDF
Enrolment Record PrintFor students who wish to print their current enrolment record. Students DO NOT need to submit a form. Click 'Print Enrolment Record' within SOLS.

For undergraduates in their final year or postgraduates in their final session that require a list of the subjects they have yet to complete.

For those students who require documentation detailing their name, degree and completion date. In some instances this is available immediately from Student Central.

For those students who require proof that they have completed a certain degree/course/subject and require more details than provided in the short completion letter.

Application for Refund of Domestic Fees in Special Circumstances

As a domestic student you should use this form if you have:
- incurred a fee which you paid up-front or deferred via HECS-HELP or FEE-HELP loan and
- the census date has passed but you are unable to continue your study because of special circumstances
and
- you want the debt for the semester remitted.

Application for Leave of Absence - International StudentsUse this if you want to take leave while you are studying here at the University of Wollongong.

Application for a Letter of Release - InternationalUse this if you are enrolled seeking release and have studied less than 6 months at the University.
All the information you need is on the online form, however, additional information can be found on the International Student page under 'Leaving the University'.
Application for a Letter of Release - InternationalFor students who have not yet enrolled and are seeking release or withdrawal from the University.

Course Withdrawal - InternationalUse this if you have studied at the University for at least 6 months.
If you have not studied at the University of Wollongong for at least 6 months, you will need to fill in the Application for Letter of Release (see above).

Application to Reduce Study LoadInternational Students are able to apply to reduce their study load as a result of compassionate or compelling circumstances.

 6. Cross Institutional & Non-Award Course Forms

Application for Cross-Institutional Study at UOWYou will be considered for enrolment at the University of Wollongong, as a Cross Institutional student, provided vacancies exist in the subject(s) for which you have applied.
University rules, as stated in the University of Wollongong Handbook, also apply to Cross Institutional students.

Application for Non-Award Study at UOWFor those individuals seeking to apply to study a subject at the University of Wollongong . As a  Non-Award student you must meet admission requirements which are determined by the faculty to which you are applying.

 7. Academic Consideration Form & Procedure

Academic Consideration ApplicationAcademic consideration is a process intended to help minimise the impact of serious or extenuating circumstances beyond a student's control. For those students wanting to apply for consideration in their attendance, assignments, exams or final marks, more information about the process can be found here.
Applications are online in SOLS.

Statutory Declaration for Academic ConsiderationStudents must use this form following a discussion with subject coordinators of subjects affected.
The document should only be signed when declaring in the presence of an approved witness (Oaths Act, 1900 NSW).
Justice of Peace are available at Student Central most times. Times and other JP are listed on the JP website.
