HRE Guidelines for completing applications

Applications for HREC approval need to include:

• The standard application form
• The Privacy addition if collecting/using health information
• Participant Information Sheet
• Consent Form
• Copies of questionnaires, interview questions, telephone scripts
• All advertising material
• Any approvals from other organisations that will be involved

There are specific guidelines to help you:
