Undergraduate Applications

Undergraduate Application Form [PDF 656kb]to start a degree or Honours. This form is for non-school leavers. If you are a school leaver, and sitting the HSC this year, you will need to apply through the Universities Admissions Centre (UAC). Some UOW degrees have additional application forms:

All applicants for Master of Clinical Exercise Physiology must submit the Selection Supplement for the Master of Clinical Exercise Physiology together with their course application.

All applicants for Master of Science (Nutrition and Dietetics) must submit the Selection Supplement for the Master of Science (Nutrition and Dietetics) [pdf 200kb] together with their course application.

All applicants for Master of Science (Nutrition/Dietetics and Exercise Science Rehabilitation) must also submit the Supplementary Information form Master of Science (Nutrition/Dietetics & Exercise Rehabilitation) [pdf 92kb]

All applicants for the Graduate Certificate in Advanced Dietetic Practice must also submit the Supplementary Information form Graduate Certificate in Advanced Dietetic Practice [pdf 90kb]

Research

For postgraduate research, also called Higher Degree Research (HDR), submit your application via Apply Online.
Contact UniAdvice if you have any issues and we can forward you a paper application. Includes HDR Scholarship Applicaiton form.

All applicants for a Research Scholarship must submit two Referee’s Reports (Online).

All applicants for Clinical Psychology research programs must submit the Personal Statement Form [doc 48kb], Clinical Psychology Referees Report [doc 66kb], and Preliminary Research Proposal Form [doc 51 kb]
