The Researcher: This research is being conducted by Jai Greenaway as part of a Doctoral Study at the University of Wollongong. The Doctoral Supervisor is Dr Ian McGrath, Faculty of Creative Arts, University of Wollongong. Jai has worked as a drama/theatre teacher and performing arts industry professional since the 1970's. Jai's current position is Drama/Theatrical Technology teacher at an Independent School in Sydney.

This research has been reviewed by the University of Wollongong Human Research Ethics Committee. Ethics Number HE02/078

The Purpose: This questionnaire is designed to collect information from NSW educators which will be used to create resources for teachers in educating their students about safety in the Performing Arts. Because of the range of respondents some of the questions may not apply to your situation.

Your Privacy: Participation in this questionnaire is entirely voluntary.

At the conclusion of this questionnaire there is a section which asks for contact details. It is entirely voluntary to provide your details but it may help the research if we can contact you to elaborate on aspects of your experiences. If you agree and provide your contact details you will be initially approached by email to discuss any further contact.

Agreement to be contacted does not make your responses to the questionnaire identifiable to you.

If at anytime you wish to withdraw your consent to be contacted or your identifiable participation in this research please email your request to jmg62@uow.edu.au. Information which does not identify the participant cannot be withdrawn. Unidentifiable data cannot be withdrawn.

Information gathered will be used in the Doctoral Research Document.

All primary data, including information which can identify participants, will be held in a secure place in the University of Wollongong with the Supervisor, Dr Ian McGrath for a period of at least 5 years.

Time: This questionnaire will take approx 15 minutes to complete.

Safety Powerpoint: At the end of the questionnaire is a copyright free PowerPoint presentation, "Safety for Student Actors" , which can be downloaded for your use. It is a template for introducing some safety issues to Drama students. It can be used in its current form or personalised to your circumstances.
